Duties of this LEAP Job Posting:
  • Facilitate healing and healthy pathways for clients
  • Develop relationships with First Nations communities
  • Collaborate with Ministry staff, KCA staff and community resources
  • Participate in community reintegration tables
  • Provide Indigenous, client-centered case management services
  • Create and maintain client file, document services, supports and referrals
  • Meet with clients regularly to assess, refer, connect and support them, and to monitor their progress
  • Assist in connecting clients with their communities and community support services
  • Assist clients with transitional and supportive housing
  • Refer clients to culturally based healers and counselling
  • Facilitate connection to the land through land-based healing
  • Assist clients to access wellness opportunities
  • Connect clients with emergency supports, as needed
  • Refer clients to appropriate KCA programming and supports
  • Provide culturally relevant supports
  • Complete and submit reports as required
  • Contribute to the evaluation of service with respect to its effectiveness in meeting the community needs and achieving our mandate
  • Additional duties as required
Qualifications for this LEAP Job Posting:
  • Post-secondary education in the mental health, social services and/or justice fields or equivalent combination of training and experience
  • Minimum 3-5 years of related experience
  • Strong time management skills
  • Ability to work independently and in a team setting
  • Excellent skills in the following areas: computer, organization, networking, interpersonal, presentation and communication (written and oral)
  • Experience working with and knowledge of Indian Residential School survivors
  • Experience with Mustimuhw cEMR (MIS) considered an asset
  • Knowledge of the Anishinaabe culture and language and experience in working in an Indigenous community or organization considered an asset
