Couchiching First Nation is seeking a permanent full-time Indigenous Transition Facilitator to join their Health department. Duties of this LEAP Job Posting:

Reporting directly to the Health Manager, this role empowers clients and their support systems to achieve independence within their rural communities. This role integrates provincial, federal and community health and social services to comprehensively address client’s physical, mental, emotional, and spiritual needs. By working closely with both individuals and their support systems, you will ensure they have the resources, care, and education they need throughout their care journey.

  • Develop personalized care plans tailored to individual needs ensuring that clients receive appropriate interventions, follow-up care, and access to necessary resources.
  • Ensure caregivers are engaged early in the care plan process and identify any necessary education and training for supporting the client at home.
  • Provide education to clients and families on health promotion and disease management.
  • Incorporate traditional practices into nursing care and advocate for access to culturally appropriate resources.
  • Coordinate continuity of care and collaborate with circle of care partners.
  • Build relationships across transdisciplinary teams and community partners to enhance transitions from acute care to community and long-term care.
  • Provide direct nursing care to clients in their homes, including wound care, medication administration, vital signs monitoring, health assessments, and immunizations.
  • Assist Personal Support Workers with complex activities of daily living when required.
  • Participate in meetings, conferences, seminars, and community health committees.
  • Maintain accurate and timely documentation in electronic medical records.
Qualifications for this LEAP Job Posting:
  • A Nursing degree or a Practical Nursing diploma from a Canadian or accredited institution.
  • Registered member in good standing with the CNO with general class registration.
  • Experience in community nursing, home care, or working with Indigenous populations.
  • Knowledge of the First Nation, culture, traditions and health beliefs is essential.
  • Strong clinical and interpersonal skills.
  • Proficient with computers with the ability to learn new software.
  • Excellent written and verbal communication skills.
  • Demonstrated ability to work independently and as part of a team.
  • Willingness to complete training and certification process for Immunization Certification.
  • First Aid/CPR Certification or willingness to be certified.
  • Valid G level driver’s licence and access to a vehicle.
  • Ability to provide a Vulnerable Sector Check.
  • Willingness to work flexible hours.
