Duties:

The Internal Medicine Clinic Nurse Practitioner (NP) provides high-quality, patient-centered care within the internal medicine clinic. This role involves assessing, diagnosing, and managing acute and chronic conditions, delivering direct patient care, coordinating with internal medicine physicians, interdisciplinary teams, community partners, and contributing to program development and patient education initiatives. The NP plays a critical role in optimizing patient outcomes and enhancing the overall experience of care in an ambulatory setting.

 

As the NP you will work closely with Internal Medicine physicians to provide longitudinal care for outpatients. This would include providing in person and virtual reassessments, ordering and reviewing investigations, prescriptions (including renewals) and titration of medications for patients with CHF, COPD, Cirrhosis, Diabetes Mellitus and other complex comorbidities. The NP will provide follow up post-discharge from the hospital to ensure continuity of care.

 

This position offers the opportunity to practice with autonomy in coordinating and managing patient care, including scheduling patient appointments, follow-up visits, and diagnostic testing. In addition, the administrative responsibilities associated with the role will be shared collaboratively between the Nurse Practitioner and Internal Medicine Clinic physicians. A comprehensive 4-week orientation and onboarding period will be provided in collaboration with the physicians, with a gradual introduction to scheduled patients and transition toward an independent Nurse Practitioner-led caseload.

 

Qualifications:

  • Preference will be given to those with nursing experience in an acute setting or relevant specialty. Other combinations of experience may be considered
  • Current registration in the NP class from the College of Nurses of Ontario
  • Master’s degree in nursing
  • Current Certification in BCLS and demonstrated ongoing competence through annual re-certification
  • Certification in IV Therapy with demonstrated ongoing competence
  • Certification in CPI or willing to obtain
  • Strong clinical assessment and diagnostic skills, proficiency in electronic medical records (EMR) systems, and a commitment to evidence-based practice
  • Excellent verbal and written communication skills
  • Sensitivity to the needs of a multicultural community and diverse populations
  • Demonstrated ability to work effectively in a multi-disciplinary team environment
  • Strong interpersonal skills with the ability to work with external and internal stakeholders
  • Reliable attendance record
