All Nations Health Partners Ontario Health Team (ANHP OHT) is seeking an experienced healthcare leader to establish and lead our new Primary Care Central Intake program.

This is a unique opportunity to build a regional service from the ground up, while helping transform how people access primary care across the Kenora region. Working closely with physicians, Indigenous health organizations and community partners, you will lead the implementation of a coordinated intake model designed to improve patient attachment, navigation, and access to primary care.

Duties:

The OHT Primary Care Central Intake Project Manager will oversee day-to-day operations of the central intake team, providing leadership to Intake Navigators and Referral & Data Coordinators, while ensuring patients are connected with the right primary care provider at the right time.

Working collaboratively with partner bodies and organizations, including the Rural Generalist Care Council, Primary Care Network, Kenora Chiefs Advisory, Sunset Country Family Health Team, Waasegiizhig Nanaandawe’iyewigamig Health Access Centre, Lake of the Woods District Hospital, and participating primary care providers, you will ensure centralized intake services are coordinated, culturally safe, equitable, and responsive to community needs.

 

You’ll also play a key role in advancing quality improvement, system planning, performance measurement, digital health initiatives, and future expansion of integrated primary care services across the ANHP OHT region.

  • Operational Leadership: Lead the daily operations of the Primary Care Central Intake Team, providing supervision, coaching, workflow management, and continuous improvement while ensuring high-quality patient service.
  • Program Implementation: Build and oversee the Primary Care Central Intake model, developing standardized intake processes, referral pathways, navigation protocols, and service standards.
  • Project Leadership: Coordinate implementation of new initiatives, facilitate cross-organizational working groups, manage timelines and deliverables, and support regional healthcare transformation.
  • Partnership Development: Build collaborative relationships with physicians, Indigenous partners, hospital, community agencies, Ontario Health, and regional healthcare organizations.
  • Quality Improvement & System Planning: Monitor patient access, referral performance, attachment outcomes, and operational metrics while leading quality improvement initiatives.
  • Data & Performance Management: Oversee reporting, dashboards, operational analytics, and performance measurement to support planning, accountability, and reporting requirements.
  • Digital Health Leadership: Support implementation of electronic referral systems, EMR integration, OceanMD, standardized documentation, and future digital health initiatives.

 

Qualifications:

  • Bachelor’s degree in Health Administration, Nursing, Business Administration, Health Sciences, Social Work, Public Administration, or a related field. Master’s degree considered an asset.
  • Five to seven years of progressive leadership experience in healthcare, preferably within primary care, hospitals, Ontario Health Teams, or community health organizations.
  • Demonstrated experience leading interdisciplinary healthcare teams and managing complex, multi-organizational initiatives.
  • Strong knowledge of Ontario’s primary care system, integrated care models, patient navigation, and healthcare transformation.
  • Experience in project management, quality improvement, performance measurement, and change management.
  • Experience working with EMRs, OceanMD, Microsoft 365, SharePoint, Teams, and healthcare reporting systems. PMP, Lean Healthcare, Prosci Change Management, or equivalent certification considered an asset.
  • Demonstrated commitment to Indigenous partnership, cultural safety, and health equity.
  • Valid driver’s licence and access to reliable transportation.
  • Anishinaabemowin language skills are a strong asset. Clear Vulnerable Sector Check required.
