All-Nations Health Partners Ontario Health Team (ANHP OHT) is seeking a highly organized and detail-oriented professional for the role of OHT Primary Care Data & Referral Coordinator.

Duties:

The OHT Primary Care Data & Referral Coordinator plays a critical role in supporting primary care attachment and coordinated care delivery across the Kenora region.

Working closely with the OHT Primary Care Intake Navigator and ANHP partners, you will help ensure patient information is accurate, referrals move efficiently through the system, and healthcare providers have the information they need to support timely access to care. You will also support data reporting, performance monitoring, and continuous quality improvement across the ANHP Ontario Health Team. You will work collaboratively with partners across the ANHP Ontario Health Team, including KCA, WNHAC, SCFHT, LWDH, and other ANHP OHT partner organizations.

  • Health records management and data quality: Support data quality initiatives, audits, and records management activities by maintaining accurate, secure, and up-to-date patient information and ensure consistency in documentation and data entry practices across partner organizations.
  • Referral tracking and coordination: Support closed-loop referral processes and timely follow-up by tracking referrals from intake through to confirmed outcomes while working with system partners to resolve documentation gaps and referral barriers.
  • Contribute to Reporting & System Improvement: Support performance monitoring and quality improvement activities by assisting with dashboards, reports, and data summaries aimed at identifying opportunities to strengthen administrative processes and workflows across the OHT.

Qualifications:

  • Education: Degree or diploma in health administration, data administration, medical office administration, or a related field (or equivalent experience).
  • Experience: 2+ years’ experience in electronic medical records, medical administration, data management, referral coordination, or healthcare administration.
  • Knowledge: Strong understanding of PHIPA and health information management best practices.
  • Cultural Grounding: Commitment to equity, inclusion, and cultural humility, with an understanding of Indigenous cultures, values, and the impacts of historical trauma. Experience working alongside Indigenous communities and organizations is an asset.
  • System Mindset: High adaptability, a solution-focused approach to process improvement, and an openness to continuous feedback and peer coaching.
  • Organization and Communication: Exceptional attention to detail and commitment to data accuracy with the ability to work collaboratively across organizations and build strong working relationships.
  • Clear Vulnerable Sector Check required.
