The Registered Nurse – Certified Respiratory Educator (CRE) provides patient-centred education and ongoing management for individuals living with asthma, Chronic Obstructive Pulmonary Disease (COPD), and other chronic conditions. This role emphasizes empowering patients through education, coaching, and evidence-based self- management strategies to improve health outcomes and quality of life. In this position, the RN–CRE is often cross-trained to support additional chronic disease programs, such as INR monitoring and hypertension management. Recognizing that many patients live with multiple co-morbidities, the role promotes a holistic, integrated approach to chronic disease management while providing patients with a consistent provider to support continuity of care.

Duties:

Patient Education & Chronic Disease Management

  • Provide individualized education, coaching, and lifestyle counselling for patients with chronic conditions, including development and review of personalized self-management and action plans.
  • Educate on inhaler technique, medication adherence, and medical device use, and support recognition of triggers, early exacerbation signs, and appropriate symptom response
  • Provide follow-up, monitoring, and ongoing support to promote sustained behaviour change, disease control, and long-term patient engagement

Spirometry Testing & Clinical Support

  • Perform spirometry testing in accordance with standards, ensure quality control, and explain the testing process and results to patients

Cross-Training & Integrated Chronic Disease Programs

  • Participate in cross-training to support INR, hypertension, and other chronic disease programs using a holistic, integrated approach
  • Coordinate care and education with the interprofessional team to support patients with complex, multiple co- morbidities

Program Planning & Quality Improvement

  • Contribute to development, implementation, and evaluation of chronic disease programs
  • Participate in quality improvement initiatives and track patient outcomes and program indicators

Interprofessional Collaboration

  • Work collaboratively with physicians, nurse practitioners, pharmacists, and allied health professionals
  • Communicate patient progress, education needs, and participate in interdisciplinary care planning

Qualifications:

  • Consideration will be given to Registered Nurses willing to obtain their CRE
  • RN in good standing with the College of Nurses of Ontario (CNO)
  • Certified Respiratory Educator (CRE) or willingness to obtain
  • Strong background in patient education and chronic disease management
  • Excellent communication, coaching, and motivational interviewing skills
  • Experience in primary care / Family Health Team / Ontario Health Team setting (preferred)
  • Experience supporting chronic disease programs (e.g., INR, hypertension) and performing spirometry (preferred)
  • Proficient with electronic medical records and quality improvement initiatives
  • Holistic, patient-centred, organized, and collaborative approach to care
