Duties of this LEAP Job Posting:
- Works collaboratively with a multi-disciplinary and ethical nursing care to ensure the personal safety of patients
- Engages patient using a collaborative approach, building rapport and trust, achieving a mutual understanding and agreement to identify actual and potential patient health care needs, strengths, capacities and goals.
- Establishes and maintains appropriate professional boundaries with patients and the health care team, including the distinction between social interaction and therapeutic relationships
- Discharges patient when goals have been achieved
- On-call is required dependent on branch __cpLocation and program
- Participates in necessary care conferences with Health Care team
Qualifications for this LEAP Job Posting:
Registration licensing with provincial regulatory college in good standing
Driver’s license and access to reliable vehicle
