Wage: $47,014- $61,342 with excellent benefits, including HOOPP pension plan. Salary is based on education and experience.
We provide primary health care through a range of options including prevention, assessment, diagnosis, and treatment. Our interprofessional teams include health professionals, educators and support workers representing both traditional Indigenous and contemporary models of care and using wholistic health and community development concepts.
This position connects people with health and social services that will result in improved health outcomes, and healthier families and communities. It is also intended to address system level challenges that impact planned transitions in care for people being discharged from a regional facility to a First Nation or other community, and bridge understanding between traditional and western approaches to health and healing with a focus on quality, equitable access and culturally safe care.
This position supports implementation of an Indigenous Transitional Care Model by working with health and social service providers to facilitate access to optimal supports for clients requiring health care services. NOTE: Indigenous traditional practices involve cultural ceremonies and burning sacred medicines including tobacco, sweetgrass, sage and cedar within the work setting.
Qualifications:
- Valid Class “G” drivers’ license and own vehicle required;
- Copy of clean driver’s abstract required (for the past 1–3 years).
- Current First Aid, CPR, and AED certification or willingness to complete training.
- Completion of Cultural Safety Training, or willingness to complete training.
- Strong oral and written communication skills.
- Demonstrated reliability, professionalism, and strong organizational skills.
- Ability to maintain participant confidentiality and professional boundaries.

- Current criminal records check required
