Duties of this LEAP Job Posting: As a member of the SCFHT, the Nurse Practitioner (NP) plays a vital role in delivering high-quality, comprehensive primary care within their full scope of practice, as defined by the College of Nurses of Ontario. The NP will assess, diagnose, plan, implement, and evaluate care for patients across the lifespan, working both independently and in collaboration with physicians and other SCFHT team members. Responsibilities include conducting health assessments, ordering and interpreting diagnostic tests, prescribing medications, managing chronic and acute conditions, providing preventative care, performing procedures within their scope (e.g., pap tests, wound care, joint injections), maintaining accurate documentation, and supporting health promotion and patient education.
Qualifications for this LEAP Job Posting:
- Current registration in good standing with the College of Nurses of Ontario as an RN-EC (Extended Class)
- 3-5 years’ experience working as a Nurse Practitioner
- Ability to work both independently and collaboratively within an interdisciplinary primary healthcare team
- Strong clinical, organizational, interpersonal, assessment, and prioritization skills
- Be able to provide proof of professional liability insurance
- Be able to provide a valid CPR certificate
- Be able to provide a Criminal Record and Judicial Matters Check for the Vulnerable Sector
- Be able to provide proof of Automotive Liability Insurance with a minimum of two million in coverage
Working Conditions
- 32 hours per week (1,664 hours annually)
- The position is clinic-based and involves direct, in-person patient care.
