Duties of this LEAP Job Posting:

As part of Ontario’s Primary Care Action Plan to connect every resident with a primary care provider, the All-Nations Health Partners is seeking an Attachment Coordinator to lead local attachment efforts across the All-Nations Health Partners Ontario Health Team (ANHP OHT). The Attachment Coordinator will play a key role in ensuring patients are connected to a consistent, publicly funded primary care provider by coordinating cross-organizational collaboration, validating patient data, and directly supporting patients in accessing care.

Key Responsibilities:

  • Maintain and reconcile accurate patient data across electronic medical records and with Health Care Connect.
  • Engage directly with patients to support attachment and connect them with education and available services.
  • Liaise with ANHP primary care providers and other patient service providers to align patients with appropriate care teams.
  • Act as the primary contact for Health Care Connect.
  • Monitor and report progress toward provincial attachment targets and local OHT attachment goals.
  • Advocate for equitable, culturally safe access to primary care across the ANHP OHT.
Qualifications for this LEAP Job Posting:
  • Preference will be given to post-secondary education in health administration, health information management, public health, social work or related work experience and skills.
  • Experience in primary care or Ontario’s health system; knowledge of enrolment and attachment processes will be considered an asset
  • Proficiency with electronic medical records and data management.
  • Strong interpersonal skills, with the ability to engage the public and health care providers effectively.
  • Experience working with Indigenous communities is an asset.
