Are you a Registered Nurse with strong leadership skills and a passion for improving health systems and patient care? The Sunset Country Family Health Team (SCFHT) is seeking a Clinical Coordinator (RN) to lead the coordination, oversight, and continuous improvement of clinical programs and services across our interdisciplinary team. In this pivotal leadership role, you’ll be responsible for driving high-quality, evidence-based care by optimizing clinic operations, supporting staff development, advancing access to care, and aligning clinical practice with organizational goals. This role is ideal for an experienced RN who is both strategic and hands-on, equally comfortable with systems planning and stepping in to provide clinical support when needed.

Wages: Annual salary $100,297, HOOPP Pension Plan and Group benefits (health, dental, STD/LTD), Starting at 20 vacation day per year; 3 personal days; 15 sick days
Duties of this LEAP Job Posting:
- Lead the development and monitoring of clinical performance metrics and outcomes.
- Oversee provider scheduling and manage clinic coverage to maintain seamless care.
- Guide professional development initiatives and evaluate continuing education requests.
- Provide nursing coverage during staff absences or high-demand periods.
- Manage vaccines and immunizations, ensuring safety and proper storage.
- Lead emergency and pandemic preparedness planning.
- Act as the Occupational Health Nurse to support staff health and safety.
- Represent SCFHT in regional and provincial healthcare planning initiatives.
- Maintain and update medical directives, policies, and procedures.
- Foster communication across SCFHT teams and promote programs within the community.
- Provide administrative and clinical support to optimize under-resourced programs.
- Coordinate all clinical student placements, including onboarding and supervision.
Qualifications for this LEAP Job Posting:
- Registered Nurse in good standing with 5+ years of clinical and supervisory experience.
- Proven ability to lead and support a multidisciplinary team.
- Strong background in clinical program planning, performance evaluation, and QI.
- Skilled communicator with excellent organizational and facilitation skills.
- Deep understanding of health system integration and determinants of health.
- Experience working independently and collaboratively within interprofessional teams.
- Commitment to continuous improvement, client-centered care, and confidentiality.
