Duties of this LEAP Job Posting:
  • Reporting to the Nursing Manager, the Home Care Nurse is responsible for ensuring that the quality of the Foot Care services to clients in the community and facility setting is compliant with legislation and standards of practice
  • Work closely with partner health care providers and consult with clients’ Primary Care Physician, visiting physicians and Nurse practitioners on an ongoing basis
  • Provide referrals to community programs as well as partner agencies and supports and act as a patient advocate when needed
  • Perform comprehensive and focused health assessments that include health history, health status, risk factors and the social determinants of health and develop client care plans
  • Conduct needs assessments for advanced care and medical directives, complete and document thorough medication assessments, review laboratory and diagnostic reports and flag potential concerns for visiting Physicians and Nurse Practitioners
  • Assist and support clients with planning, testing, transportation and in-home care in accordance with the First Nation and Inuit Home and Community Care Program guidelines
  • Provide follow up care and consultations for clients, coordinate services and care with clients and help resolve client concerns
  • Provide interventions as per medical directives or medical orders, such as wound care, staple/suture removal, injections, ear syringing, assisting with procedures and specimen collection in accordance with Ontario College of Nurses Standards of Practice
  • Screen and monitor to prevent the onset of health issues and/or minimize complications and provide education and support regarding health promotion and prevention
  • Perform program evaluation and reporting, including collection and compilation of data in accordance with funding requirements, entering information into Community Based reporting templates, e-SDRT and E-HRTT, preparation of reports for community leadership and preparation of proposals when needed
  • Engage with individual KCA member community leadership and health representatives to conduct needs/assets assessments
  • Negotiate and assist individual First Nations communities with advocacy for equitable services and agreements with federal, provincial and regional service delivery partners in order to enhance Community Health Nurse and Primary Care Nurse programming.
Qualifications for this LEAP Job Posting:
  • Certification Registered Practical Nurse in the province of Ontario with good standing and membership to RNAO with LAP
  • Current CPR/AED with HCP and First Aid certification
  • Understanding of and ability to apply Clinical Practice Guidelines as well as other acts and legislation that govern practice
  • Experience in program planning and evaluation and performance management
  • Experience and understanding of Indigenous cultural issues, the geographical realities and social conditions within northern remote First Nations communities
  • Knowledge of the Social Determinants of Health specific to First Nations Communities and experience in Primary Health Care
  • Demonstrated understanding of PHIPA and ability to maintain client confidentiality
  • Excellent organization and computer skills
  • Strong written and verbal communication skills and effective organizational, critical thinking and problem-solving skills
  • Knowledge of the Anishinaabe culture and language considered an asset
  • Ability to work both independently and as a team member

Position Requirements

  • Valid driver’s license and vehicle;
  • Willing to travel (when required) to any Treaty 3 First Nation communities;
  • Current satisfactory criminal reference check with vulnerable sector search;
  • Understanding of the language of the Anishinaabe language (Ojibway) is an asset.
